Zepbound® (tirzepatide) is a once-weekly injectable prescription medication indicated for chronic weight management. The 2.5 mg/0.5 mL dosage serves as the initial titration dose, designed to allow the body to adjust to the medication’s physiological effects while minimizing the incidence of gastrointestinal adverse reactions.
Product Specifications
-
Dosage Strength: 2.5 mg per 0.5 mL.
-
Presentation: Single-dose vial (Note: Administration from vials requires the use of sterile syringes and needles).
-
Indication: Chronic weight management as an adjunct to a reduced-calorie diet and increased physical activity.

Important Safety Information
WARNING: RISK OF THYROID C-CELL TUMORS In rodent studies, tirzepatide has been shown to cause thyroid C-cell tumors. It is unknown if this occurs in humans. Zepbound is contraindicated in patients with a personal or family history of medullary thyroid carcinoma (MTC) or in patients with Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).
Clinical Considerations for Initial Titration
-
Dose Escalation: The 2.5 mg dose is intended to be used for 4 weeks. Following this period, the dosage is typically increased to 5 mg weekly to achieve the target therapeutic effect, as 2.5 mg is not intended for long-term glycemic or weight management maintenance.
-
Gastrointestinal Tolerance: Patients should be monitored for nausea, vomiting, and diarrhea. If these symptoms become severe, they should be reported to a healthcare provider.
-
Injection Technique: Because this presentation is a vial, patients must be instructed on the proper use of sterile syringes to ensure an accurate 0.5 mL dose is drawn and administered.
-
Contraindications: History of MTC/MEN 2; known hypersensitivity to tirzepatide.
Storage and Handling
-
Refrigeration: Store vials in the refrigerator at 36°F to 46°F (2°C to 8°C).
-
Room Temperature: If necessary, the vial may be kept at room temperature (up to 86°F or 30°C) for up to 21 days.
-
